For sciatica patients in Hollywood, Hallandale Beach, Pembroke Pines, Fort Lauderdale, and across Broward County, non-surgical Class IV laser therapy is one of several conservative care options to consider before pursuing spinal procedures or long-term medication use. This is the clinical conversation that often happens late in a sciatica patient’s treatment journey, after months of standard care.

What Is Sciatica and What Is Actually Causing the Pain?

Sciatica describes pain radiating along the path of the sciatic nerve, which originates from nerve roots at the L4, L5, S1, S2, and S3 levels of the lumbar and sacral spine and travels through the buttock and down the leg, sometimes reaching the foot.

The pain is generated by compression or chemical irritation of one or more of those nerve roots, most commonly from a herniated lumbar disc, spinal stenosis, or degenerative disc disease. The American Association of Neurological Surgeons estimates that approximately 85 percent of Americans will experience back pain at some point in their lives, and lumbar radiculopathy is among the most common disabling presentations.

A 2015 study in the European Spine Journal found that lumbar disc herniation at L4-L5 and L5-S1 accounts for the majority of clinically significant sciatica cases. Piriformis syndrome (compression of the sciatic nerve by the piriformis muscle in the buttock) is a non-spinal cause that mimics disc-related sciatica and is frequently missed in standard evaluations. South Florida’s active population, including cyclists, golfers, and paddlers, places significant demand on the piriformis and hip external rotators, which makes this presentation common in patients I see across Broward County.

Why Does Sciatic Pain Persist Even After Treatment?

The persistence of sciatica in many patients is explained by a mechanism that conventional treatment rarely addresses, the inflammatory nerve root environment. Research published in Spine confirmed that herniated nucleus pulposus material triggers an immune-mediated inflammatory response at the nerve root that amplifies pain signals independently of mechanical compression.

A 2014 Cochrane Review found that epidural corticosteroids provided only short-term, modest benefit for sciatica, with no significant difference in outcomes at 12 months. The structural cause, the disc compressing the nerve, remains unchanged.

How Does Class IV Laser Therapy Work?
